PROCEDURES TO REINSTATE LAPSED INDIVIDUAL
When the records of a licensee are placed in
the retention file by request, for failure to renew, or for lack of
correct address, the following procedure is followed:
The maximum holding time for the
retention file is three (3) years. Records are destroyed after
three years. If registration is desired after three years, a new
application must be submitted to the Board for approval.
To reinstate the license any time during
the 3-year period, the following is required:
1. Complete the 'Application to reinstate
license'. You can complete the application
online and submit with a credit card OR mail the application with a
check or fax your credit card information with a
Credit Card Authorization Form found on our website. Reinstatement fee is $180.
2. Complete the online open-book
Take Home Exam over South Dakota State Laws
found on our website. Answers can be found under our
Blue Book (Statutes and Rules).
3. If the business you work for is not currently licensed
with our office, complete a
found on our website.
PDH Log by mail, fax or scan and
email to firstname.lastname@example.org.
This log can be found on our website. This indicates that you have
completed 30 hours of continuing education during the previous two (2)
years from the date of submitting this application to reinstate.
