There will be additional documentation required for the new, extended PUA program effective Dec. 27, 2020.

You will need to provide proof you were:

  • working or self-employed at the time of the COVID-19 pandemic, or
  • scheduled to start working on or after the start of the COVID-19 pandemic.

Proof of employment/self-employment or commencement of employment/self-employment (required)

Acceptable documentation:

  • Proof of employment includes, but is not limited to, paycheck stubs, earnings and leave statements showing the employer’s name and address, and W-2 forms when available.
  • Proof of the planned commencement of employment includes, but is not limited to, letters offering employment, statements/affidavits by individuals (with name and contact information) verifying an offer of employment.
  • Proof of self-employment includes, but is not limited to, state or federal employer identification numbers, business licenses, tax returns, business receipts, and signed affidavits from persons verifying the individual's self-employment.
  • Proof of the planned commencement of self-employment includes, but is not limited to, business licenses, state or Federal employer identification numbers, written business plans, or a lease agreement.
You may be asked for proof of identity.
If you do not provide this required documentation, you may be held overpaid for some or all weeks of benefits you have received.

Proof of earnings (if available)

(For prior PUA claimants who have not submitted earnings or first-time PUA claimants.)

If you're unable to provide proof of earnings, you will only be allowed the minimum PUA benefit payment of $172. If you provide your proof of earnings, your claim will be redetermined. If it results in a higher amount, adjusted payments will be issued to you. If your claim has an effective date prior to Jan. 3, 2021, please provide one of the following if able and you have not previously done so:

  • 2019 Federal Tax Return (preferable)
  • 2019 1099 - Miscellaneous Income Only
  • 2019 W-2
  • Final Pay stub in 2019

If your claim has an effective date of Jan. 3, 2021, or after, please provide one of the following if able:

  • 2020 Federal Tax Return (preferable)
  • 2020 1099 - Miscellaneous Income Only
  • 2020 W-2
  • Final pay stub in 2020
